When patients ask me about Motiva Preservé, the conversation usually starts the same way: they have done their research, they know they want a natural result, and they want to understand exactly what separates this implant from everything else currently available. That is a sophisticated starting point, and it deserves a precise answer. What follows is my attempt to give you one.

What Motiva Preservé Is

Motiva Preservé is a next-generation cohesive silicone gel breast implant manufactured by Establishment Labs, the same company behind the widely recognised Motiva Ergonomix line. The Preservé shell is constructed with a proprietary SmoothSilk micro-surface technology, a surface finish so refined that under scanning electron microscopy it approaches the smoothness of native tissue. That surface characteristic matters clinically because textured implant surfaces have been associated with BIA-ALCL (breast implant-associated anaplastic large-cell lymphoma), a rare but serious condition that the FDA has asked surgeons to discuss with every augmentation patient. SmoothSilk carries no documented association with BIA-ALCL.

The gel inside Preservé is a shape-stable yet responsive cohesive silicone. It holds a gentle anatomic profile when you are upright, and it redistributes naturally when you lie down or move, in a way that closely mimics breast parenchyma. The shell incorporates Motiva's BluSeal indicator layer, a thin barrier film that gives both surgeon and radiologist a visual and tactile confirmation that implant integrity is intact at every stage from manufacturing through implantation.

Every Preservé implant also contains a QInside Safety Technology RFID microtransponder embedded in the shell. That chip stores the implant's unique serial number and can be read with a clinical scanner in seconds, giving your medical team instant, accurate device identification throughout your lifetime without depending on wallet cards or paper records.

The SmoothSilk surface, the BluSeal integrity layer, and the embedded RFID chip are not marketing features stacked for the brochure. Each one addresses a distinct clinical problem, and together they form a safety architecture that holds up at every stage of the implant's life.

How Preservé Differs from Traditional Breast Augmentation

You may have heard the results described as the "ballerina breast aug" or "yoga boobs," shorthand for a silhouette that reads as athletic and unforced rather than round and static. Preservé builds on that foundation, but with meaningful refinements.

Preservé is an innovative breast tissue preservation augmentation technique, using Motiva Ergonomix implants, which allows breast tissue to be expanded with a specifically designed balloon rather than cut resulting in - quicker recovery, reduced pain and smallest scar.

Gel Cohesivity

Ergonomix implants use a lower-cohesivity gel optimised for maximum positional movement. Preservé uses a moderately higher-cohesivity gel that still moves naturally but maintains a more defined upper-pole contour. If you want a softer, almost teardrop shape that relies almost entirely on gravity for its profile, Ergonomix may suit you. If you want reliable upper-pole fullness that travels with you through every posture, Preservé is typically the stronger option.

Shell Architecture

The Ergonomix implant shell used in Preservé features Motiva's TrueMonobloc technology, meaning the shell, barrier layer, and gel are engineered as a single integrated unit rather than bonded layers. In practical terms, this construction is intended to reduce the micro-folding stress points that can, over many years, contribute to shell fatigue.

Intended Patient Profile

Preservé technique was developed with a more selective candidacy profile in mind, including patients with limited native breast tissue and those whose anatomy and aesthetic goals call for a more precisely controlled upper-pole shape. Your surgeon will assess whether your soft tissue envelope and your desired outcome place you in the Preservé profile or suggest a different device.

Who Is a Candidate

From an anatomical standpoint, Preservé technique tends to be most appropriate for patients who meet several criteria. Below is how we frame candidacy at our practice.

Soft Tissue Coverage

Because Preservé technique uses a smooth-shell implant, adequate soft tissue coverage is important to mask any palpable implant edges. Patients with a pinch thickness of at least 2 cm in the upper pole are typically well-suited. For patients with very thin coverage, we discuss whether submuscular or dual-plane placement can supplement the native tissue envelope.

Chest Wall and Breast Base Dimensions

Ergonomix implants used in Preservé are available in a range of base widths. Matching the implant's base width to your measured chest wall ensures that the device sits within its anatomic footprint, which is the foundation of a natural result. Your preoperative measurements, taken in our office, drive that selection.

Aesthetic Goal

If your goal is a subtle, proportionate enhancement, what patients sometimes describe as a result that looks like you "always had it," Preservé is worth a serious look. If you are seeking a more dramatically projected, visibly augmented silhouette, a different technique may align better with that goal.

General Health and Realistic Expectations

You should be in good general health, a non-smoker or willing to stop smoking for a defined period before and after surgery, and at a stable weight. Augmentation is not a weight-loss procedure, and if your weight fluctuates significantly after surgery, your long-term result can be affected regardless of which implant you choose.

Consultation

Your consultation is where the technical planning happens. Expect your surgeon to take detailed breast measurements (base width, nipple-to-inframammary fold distance, upper and lower pole pinch thickness), review your health history, and discuss your goal images or descriptions in specific terms. We use sizer trials where appropriate so you can see a volume approximation before committing to a device.

During this visit you will receive a thorough discussion of the known risks of breast augmentation, including capsular contracture, implant malposition, and the current regulatory landscape around breast implant illness and BIA-ALCL. Choosing a SmoothSilk device does not eliminate all surgical risk, but the surface-specific BIA-ALCL concern that applies to textured implants does not apply to Preservé.

Surgical Planning

The inframammary fold incision is most used with Preservé because it provides direct visualisation of the pocket and allows precise implant positioning. The type of breast augmentation thats right for you is based on your anatomy, goals and your surgeon's assessment of what will give the cleanest pocket and the most inconspicuous scar.

The Procedure

Breast augmentation with Preservé can be performed under local anaesthesia as an outpatient procedure. You will go home the same day with a supportive surgical bra and written discharge instructions, and your surgeon will review the expected operative time with you during your preoperative planning.

Recovery Expectations

Your recovery after Motiva Preservé augmentation follows the same general arc as breast augmentation with any modern cohesive silicone implant, though individual experiences vary.

The First Week

Expect moderate tightness across the chest and some swelling. Most patients manage discomfort with prescribed oral analgesics for the first two to four days, then transition to over-the-counter medication as the tightness eases. During that few days, plan to rest, keep your arms below shoulder height, and avoid any activity that raises your heart rate significantly.

Weeks Two through Four

Preservé technique allows for a rapid recovery and most patients return to desk work and light daily and social activities within a few days. You should still avoid strenuous exercise, heavy lifting (anything over 10 pounds / 4.5 kg), and sleeping on your stomach. Your surgeon will clear you for progressive activity based on how your pocket is healing.

Month Two Onward

You will have follow-up appointments at one week, four to six weeks, and three months. Long-term, we recommend annual breast health surveillance (including imaging as appropriate to your age and risk profile) and routine clinical check-ins every few years. The RFID chip in your Preservé implant means your device can be identified accurately at any of those visits, or at any emergency department, without relying on paper records.
